Self-Management Support in Leeds for Older people and people living with Long Term Conditions

Older people and people living with long-term health conditions are at an increased risk of severe illness from Covid-19 and that means that they may need a little extra help to manage their health at home.  A directory of resources has been developed which provides links to relevant services (and information on how these may have been changed or adapted as a result of Covid-19) as well as information on online tools, virtual groups and digital support for people to access and support them with their health and wellbeing.

It’s also really important that if people have concerns about an existing health condition, are feeling unwell, or need advice around medication they still get in touch with their GP in the first instance and guidance is available through ‘your NHS is still here for you when you need us’.

Active Leeds has launched Healthy At Home with great resources to help you stay active indoors. They include workout routines for children, adults, and older adults, but also wellbeing tips. 

Sport England: How to stay active while you’re at home

Sport England have compiled some of the most useful tips on how you can get active when you’re at home, if you’re well enough, including links to apps and exercise videos. General advice as well as specific videos for older people.
